Permission Form
A permission form is needed for this event. Please click on the highlighted words to download and print out the permission form. Please fill out the permission form and turn it in to the Youth Ministry office located in the basement of the parish center, behind the church. If no one is in the office, please slip it into the mailslot located in the door. If you are unable to drop it off ahead of time, please make sure to bring the completed permission form, signed by your parent or legal guardian, with you the day of the event.
